Incidents and hazards
We investigate incidents to understand safety hazards in BC and what actions can be taken to manage those hazards. These incidents must involve a regulated product, which is a piece of equipment or system that is subject to the Safety Standards Act.

Make a report
Incidents and hazards can be reported 24 hours a day, seven days a week by completing our incident form or by calling 1-866-566-7233.
Please note: Railway-related incidents must be reported by first calling 1-866-566-7233. The incident form cannot be used to report railway-related incidents.
Duty holder reporting forms
If you perform regulated work or represent a company that owns regulated products, you are a duty holder. Duty holders have specific requirements for reporting incidents and hazards:
- Amusement Devices
- Boiler, Pressure Vessels & Refrigeration
- Electrical
- Elevating Devices
- Gas
- Passenger Ropeways
- Railways
